Best Practice 

The Person-Centred Practices (PCP) process is typically facilitated by the person who has a disability, along with a close network of family and friends (a ‘circle of support’) that invites the participation of professionals as needed.  This operates in contrast to traditional planning processes, which are facilitated by case managers and other categories of service worker. There is a variety of approaches to PCP— e.g., personal futures planning, essential lifestyle planning, making action plans (MAPS), planning alternative tomorrows with hope (PATH), and group action planning (GAP).

Article 25.  Universal Declaration of Human Rights 

Everyone has the right to a standard of living adequate for the health and well-being of himself and of his family, including food, clothing, housing and medical care and necessary social services, and the right to security in the event of unemployment, sickness, disability, widowhood, old age or other lack of livelihood in circumstances beyond his control.

17. One of your client’s goals is to become more independent travelling to and from work.  The client has undertaken this once and unfortunately there was a train breakdown which took away their confidence.
