The exact medical practice problem, in this case, would be dysphasia.

It’s essential to ensure PICOT is based on a nursing practice problem because it will help you narrow down to the most critical issues the patient has. As nurses, we can identify so many issues that a patient may have, but in regards to the PICOT format, we can do it one issue at a time to ensure quality patient care. Once we identify the issue, we have to make sure that it pertains to the nursing diagnosis cause it can be easy to get sidetracked and go to solve the medical problem. PICOT stands for Problem, Intervention, Comparison, Time, and Outcome. This format will help us determine the research questions we are asking and how the approach will be most appropriate. An example of a nursing practice problem when I worked at the hospital was seeing readmissions into the hospital after being discharged. This is because the ER spends less time with patients and may miss meaningful opportunities such as discharge instructions or even providing the proper nursing education to avoid readmissions. If we can find out why this happens, we can prevent future readmissions and provide the quality care that should be given in the first place.
