May has been inpatient for 5 days in the Medical ward following a fall. 

Although there are no significant physicalinjuries, May has been assessed by the multidisciplinary team and there is a provisional diagnosis of cognitive decline (possibly dementia) May has been confused since admission, but some improvement was noted in the last 24 hours. 

She is incontinent of urine, and needs prompting and assisting with hygiene. Her son has arrived from interstate; there is no other family and May’s friends have either passed away or are in care. 

There is to be a case conference latertoday in order to commence discharge planning. 

May will be quite vulnerable on her return to home. Her son will be unable to stay for very long, and has indicated May wishes to remain in her own home for as long as possible.

 

As May lives on the Gold Coast, what supports will be available for her to access? Her son is concerned about leaving her, but seems increasingly frustrated with May’s declining memory and inability to follow his conversation. He has also been angry with staff, and has made several complaints about May’s care while inpatient.

 

The Multidisciplinary Team reviewed May and determined the need for either Commonwealth Home Support, community home care or permanent residential care. What information could you give May’s son that will support both his mother and himself?
As a team, identify possible health goals for May.  How can you evaluate May’s progress in achieving these goals?  What members of the interdisciplinary health team would benefit from this knowledge?
