PROCEDURE

RATIONALE

1. Prior to performing the procedure, introduce self and verifythe client’s identity using agency protocol. Explain to the client what you aregoing to do, why it is necessary, and how he or she can participate. Discuss how the results will be used in planning further care or treatment.

2. Perform hand hygiene and observe otherappropriate infection prevention procedures.

3. Provide forclient privacy.

4. Inquire if the clienthas any historyof the following: past problems with lumps or bumps, itching, scaling, or dandruff; history of loss of consciousness, dizziness, seizures, headache, facial pain, or injury; when and how any lumps occurred; lengthof time any other problemexisted; any known cause of problem; associated symptoms, treatment, andrecurrences.

5. Inspect the skull for size, shape,and symmetry.

6. Inspect the facial features (e.g., symmetry of structures andof the distribution of hair).

7. Inspect the eyes for edema or hollowness.

8.Note symmetry of facialmovement. Ask the client to elevate the eyebrows, frownor lower theeyebrows, close the eyes tightly, puff the cheeks, andsmile and show the teeth.

9. Document findings in the client recordusing printed or electronic form or checklists supplemented by narrative notes when appropriate.
