Patient Profile:

 Patient is a 27-year old female and works nightshift in a bank company.

Pertinent Past Medical History:

Patient suffers chronic migraine, for which she was treated with Topamax®. No history of cardiovascular diseases.  

History of Present Illness:

 Patient went for a hiking excursion with friends starting early in the morning. Patient claimed to be properly hydrated during the hike. After an hour of hiking, patient began experiencing breathlessness. Few minutes later and she was already hyperventilating which caused her chest pains. 

Laboratory Tests Requested:

Electrocardiogram (ECG) 

Chest X-ray 

Blood Test

Lab Results: 

– No abnormality observed in ECG and chest X-ray.
