Kevin is a 38 year old black male with chronic back and leg pain from a severe motorcycle accident that he was in 3 years ago. In the past, he was operated on by a local orthopedic surgeon and was placed on PRN oxycodone. He states overall his pain is fairly well controlled and he is able to continue his work as a teacher. He presents to the office today complaining about worsening right sided back pain that has been present for the past 2 days. He has also had nausea which he states is normally not present. He denies any known new trauma at this time though he states he does work out at the gym 4 times a week and started lifting more weight this past week. Upon examination, vital signs show Temp 99.9, HR 101, BP 123/86, RR 18, O2 sat 98% on room air.
