Jason has just started his new role as case manager at a not for profit children’s foster care organisation. His boss, Phillip, employed Jason on a probationary period of 3 months after he received a referral from a friend to employ Jason to fill the case manager position within the organisation.
It has been nearly a month since Jason started working at the not for profit children’s foster care organisation and Phillip has recently been informed by another case manager that, upon a number of occasions after caring for a toddler with a cold, Jason does not wash his hands.
This is particularly concerning as Jason has reportedly been off work on 2 occasions with suspected flu and cold like symptoms.Â
Some of the staff who are aware of this issue are concerned about infection transmission to other children and staff when a person is not practicing hand hygiene and applying PPE.Â

Phillip is surprised with this news as Jason was provided with a thorough workplace induction which included standard and additional precautions when working with children. The organisation has policy and procedures on hand hygiene practices, posters on the wall and hand sanitiser solutions can be found in every room. Unfortunately, there has been no way of making sure that staff are following the guidelines.
Phillip decides to conduct a direct observation of Jason working with a child who has a green coloured runny nose.Â
Jason is carrying out an assessment on the child, he notices the child’s runny nose and wipes the nose with a tissue. Before the session has finished, the child grazes their knees after a fall and Jason quickly applies a band-aid to stop the small amount of bleeding from the grazed knee. After concluding the first aid treatment, Jason immediately moves to assess another child during some physical play activity.

Through direct observation, Phillip is able to confirm that Jason did not wash his hands both before and after attending to a runny nose of a child and application of a band-aid to the knee.Â
Phillip feels it is necessary to review the organisations current hand hygiene policy and provide workers with the ‘5 Moments for Hand Hygiene’ program, and feels that this would be a great initiative for the service to follow as a benchmark against hand hygiene standards and practice. After completing his own review and assessment, Phillip will seek the input and consultation from other key members in the workplace to update the policy.

Fifi is the newly inducted aged care support worker in a regional Aged Care Residential Service.
Last week, Fifi noticed that the wheelchair she was using to transfer a client from the carpark into the residential service had a faulty brake and was very awkward to stop it from rolling. Fifi mentions the wheelchair issue to her colleague Dale, who agreed that everyone finds the brakes on that particular wheelchair difficult to stop it rolling and only last week he injured himself when transferring a client.

Fifi following her duty of care mentions the wheelchair issue to her manager, Chris, and asks why something has not been done about the wheelchair as it is a hazard with a high risk of injury. Chris advises Fifi that this is the first he has heard of the issue, clearly staff aren’t following their duty of care in reporting hazards to him.Â
Chris discusses the lack of WHS reporting at the next staff meeting, and he is surprised to discover that very few of the workers are aware of their duty of care when it comes to WHS polices relating to hazard identification, risk management and incident and injury record keeping.
Chris has determined that he needs to develop a WHS action plan taking account of priorities and training needs, and implement new safety control measures in the workplace.Â
