Thomas Cornell is 80 years old and living at home with his 78-year-old wife. Born in England, he immigrated to Canada with his family when he was 3 years old. He grew up in a small farm community in British Columbia with seven brothers and three sisters. He was the oldest son of his siblings. Though he had hoped to become a health care worker, after completing his 8th grade education, he stopped school to help his father on the farm. At the age of 30 he married a long-time friend and they subsequently had five sons.
Devoted to his family, he worked hard as a farmer all his life. His children remember waking up early to help milk the cows before they went to school. Thomas was also an accomplished tailor and made handsome suits for his sons when they were small. A favourite pastime in the winter was curling.
Mr. Cornell was diagnosed with dementia ten years ago. He is hard of hearing, wears glasses, has high blood pressure and is often constipated. Currently, his dementia is moderate; however, his wife reports that Mr. Cornell has been having increasing difficulty with activities of daily living. He has difficulty getting dressed and often puts his clothes on backwards and inside out. He becomes easily angered and aggressive when anyone suggests that his clothes are not put on properly.
Although Mrs. Cornell is very devoted, her own health is deteriorating, and she is physically unable to care for her husband at home any longer. Mr. Cornell spends hours walking around the house and on a few occasions has been found wandering the streets. Meal times are especially difficult as he often refuses to sit at the table long enough to complete a meal, resulting in a 15-pound weight loss in the last 2 months. This is of concern to his family. Upon your first visit as a PSW, you notice he repeatedly states he needs to get back to the farm to milk the cows and complete his chores.

Part A
Identify 2 behaviours that you would FLAG and REPORT to your supervisor and discuss why these would be important to FLAG and REPORT.
For each behaviour you have identified, indicate three (3) supportive care strategies or interventions you would use to assist your client. The care strategies should be described in detail and be specific to Mr. Cornell and should be within the role of the PSW.
Part B
When dementia clients are given meaningful activities (i.e. activities that they previously engaged in and are familiar with) their dementia symptoms often decrease.
From the information in the case study, determine 1 meaningful activity you would implement for your Mr. Cornell. Meaningful activities are activities with which the client is already familiar. Remember dementia clients cannot learn new information.
For the activity:
Describe the activity. What is it? Where it would take place? How would you engage the client? Who else may be involved, if anyone? What would be your role?
Explain how the resident would benefit from this activity.
