Steve is a 75-year-old man admitted three days ago to the residential aged care home you work at after a significant stroke 4 months ago. The stroke has left Steve with mobility deficits and difficulties with grip and dexterity in his hands, and he has lost a lot of weight since the stroke and now appears very thin. Steve’s wife Sally is supportive of Steve’s decision to move into residential care and Sally visits Steve daily for several hours.
Yesterday, Steve was eating his breakfast of scrambled eggs on toast at the dining room table when he started to cough and turn red in the face. Staff assisted him to remain calm and Steve was able to cough up a small piece of toast which had got stuck in his throat. At lunchtime Steve again started to splutter and gag when eating his vegetable soup and was able to tell staff that it had “gone down the wrong way.” After these incidents were reported to the nurse in charge, the nurse arranged for a speech pathologist to attend Steve for a swallowing assessment.
Today you are working in the area Steve resides in and you have been assigned to attend Steve’s personal care. In handover you are informed that the speech pathologist is due to attend to assess Steve at 11:30am and to make sure he is ready before then. Sally will also be in attendance for the assessment. You go to Steve’s room and let him know that the speech pathologist will be attending this morning, but he refuses to see her, stating he doesn’t know what that involves but he won’t do it. Steve says he won’t talk to anyone about his food because he doesn’t want to be told he can’t have scrambled eggs on toast anymore as that is his favourite meal.
