Case Study
Jim Cruz who is 65 years old is the caregiver of his 70-year-old wife Maria. Jim and his wife did not complete high schools and worked in factory jobs for over 40 years. They have no children and none of their family live in Ontario. Ten years-ago the factory they worked in unexpectedly closed. They live in a rented small house and struggle to pay the bills. They are living day to day and trying to manage on their limited income. Maria has congestive heart failure and coronary heart disease that has caused increasing fatigue, SOB, and angina-type pain over the past 3 years. Her health has steadily declined and she has lost weight. Maria states she frequently has “colds and coughs” which has led to her need to use portable oxygen in their home. She cannot climb the stairs in their home because of her exhaustion. Jim has converted the living room into a bedroom where she sleeps. She uses a wheelchair for mobility. Jim placed a commode in the living room. Jim has difficulty transferring Maria from the bed to the wheelchair or commode. He is worried about her safety in that she can fall and hurt herself. He is afraid if she fell he could not get her up.
Jim tells the nurse that he has suffered a “number of small strokes” over the past year. He has a herniated disk. He saw a physician but did not fill the prescription but rather he is taking herbal supplements (gingko biloba to prevent clots; garlic to decrease cholesterol and BP; willow bark to prevent clotting). He has refused back surgery because he says his wife needs him.
Jim says he feels alone in caring for his wife as his friends are “passing away.” He says if his family lived closer they would help out. He feels “hopeless and helpless.” He says he is sometimes short tempered and very tired. He is upset because he thinks he may not be able to keep his wife at home. However, he promised her he would never place her in a nursing home, stating “family takes care of family.”
