Scenario 3 – Discharge – Sam

 

Create  care plan for the below mentioned scenario.

 

Sam is a seventy-three-year-old man who has been in hospital for the past three weeks following a fall

at home. He has suffered a fractured femur and has trouble communicating as English is his second

language. He is ready to be discharged and is having a care plan arranged for him.

Sam lives alone and does not have any family that could support him. Although he no longer requires constant medical

assistance at the hospital, he will require support at home from other services. Sam volunteers at a local school two days

a week, where he helps young children between the ages of five and eight learn how to read. He feels that they have a

good rapport as his English isn’t great, but they help him and he helps them.

Sam wants to return to work as soon as possible as this is something he loves to do.

Needs may include:

Carers coming into the house
Help making meals
Physiotherapy/Occupational therapyï‚·
Returning to work.
