Mrs. Mays, 72 years old, has osteoarthritis, hypertension, and diabetes mellitus. She was recently hospitalized because she developed pneumonia, and her blood glucose levels were very high because of her acute illness. Currently, her pneumonia is resolving, and her blood glucose is more stable. She is preparing to go home in the next 24 hours. She lives at home with her husband, who helps her with her activities of daily living (ADLs).

Matt is a nursing student assigned to care for Mrs. Mays and prepare her for discharge home. Matt asks Mrs. Mays, “What is your main concern about going home?” Mrs. Mays responds, “My pneumonia is improving, and I know how to manage my diabetes. The pain in my hips, knees, and hands is so bad right now, that I am not sure how I will get around the house and take care of myself. My pain is so severe I have only been able to walk short distances and the pain wakes me up two or three times a night while I sleep.”
