Social determinants of health (SDOH) have been shown to drive up to 80% of health outcome for patients (RWJF, 2019). Learning about SDOH will help prepare you to understand the most common SDOH in your community, which will then help prepare you to advocate for patients and community members to minimize and potentially eliminate any health disparities that result from the SDOH.

Research social determinants of health indicators from at least 3 states (New Jersey, New York and Colorado)
