“Hello, my name is Finn (ED nurse). I am calling to give you report on Mrs. Fiona Lane. Patient is an 85-year-old female who arrived to ED from home with her daughter. Patient had an unwitnessed fall this morning and complained of severe right hip pain. Hip x-ray shows right hip fracture is present. Mrs. Lane has rheumatoid arthritis, endocarditis, hypertension, hyperthyroidism, type II diabetes mellitus, duodenal ulcer and glaucoma. She is allergic to Penicillin. Full code. Attending MD is Dr. Lisa Williams. Her recent vital signs are stable and she is on room air. AOx4. No head injury. On carbohydrate-controlled diet and 2000 mL fluid restriction. Voids normally, but has a PureWick in place. On bedrest. Has a 3-lumen PICC line on her right upper arm with NS infusing at 100 mL/hr. Labs have been drawn in ED and results are available for review. MD plan is pain management, observe hip precaution, and surgery tomorrow.”

 

Before Mrs. Lane arrives to your unit, you have a few minutes to browse through her abnormal lab results: 

Complete Blood Count (CBC) with Differential 
 

Basophil: 4% (reference range: 0.5-1%)  

Complete Metabolic Panel (CMP)
 

Glucose: 195 mg/dL (reference range: 60-140 mg/dL nonfasting)

 

After you perform a head-to-toe assessment on Mrs. Lane, you notice that the NS is infusing at 100 mL/hr as ordered (2400 mL per day), but the patient is on fluid restriction of 2000 mL per day.
