Patient came to the clinic for dizziness and heart burns.
As I am entering the patient room, I see a black female patient sitting on a recliner. The
room contains an IV pool an IV pump and a cane as well. The patient has an IV in her left
forearm that is intact and not swelling. Her skin tone is even but she has visible cut on her right
arm. Patient appears to be between 40 to 45 years Old. She looks healthy. Her hair is short and
well groomed. Her fingernails are long and well maintained. She is wearing cologne and jewelries.
No signs of acute stress are present.
Her height and weight appear to be appropriated for her apparent age range.
She appears to be well nourished, and she seems to work out.
She leans back in the recliner with her feet up and facing the TV while talking with her friends on
the phone. She is well articulate and appears to have a great sense of humor.
She reaches to get her bag from the table with no sign of discomfort. However, when she got up
to used the bathroom, she grabs her cane, claiming that she has vertigo sometimes so she wants
to be safe at all time. Overall, she is stable and able to follow the examination with full
attention. she did no involuntary movements no body twitches were Noticed. She has no sign of
physical abnormality or defects as well.
Her behavior was well suited. She answered questions with no problem. She didn’t make
much eye contact while talking to me even though she was very comfortable and pleasant.
