A 56-year-old man comes in for a routine health maintenance visit. He is new to your practice and has no specific complaints today. He has HTN for which he takes HCTZ and he admits that he occasionally takes a baby aspirin because he read somewhere that it is a good thing to do. He has no other significant medical history.
He tells you that he does not smoke, drinks alcohol socially and does not have a set exercise regimen.
His father died of a heart attack at age 60 and his mother died at age 72 of cancer. He has two sisters and both are in good health.
You examine the patient:
BP 130/80 AP 75Â Ht: 6 ft wt: 200lbs
No other clinically significant exam findings were noted during his physical.
You order a fasting lipid panel (and magically) you have the results immediately:
Total Chol-242 mg/dL
Triglycerides 138 mg/dL
HDL- 48 mg/dL
LDL 155 mg/dL
Â
For this patient, what is his LDL goal and explain how you determined that. Usse references
What laboratory testing would you order, why and what would you be looking for?
What patient teaching would you provide for this patient in terms of lifestyle changes?
What is your plan of care for this patient? Include pharmacological and non pharmacological interventions with supporting evidence.
