In the following scenario, please make 2 (two) Nursing Care Plan using ADPIE with atleast 5 interventions with rationales in each care plan. 

 

Robert Paul is a 70-year-old retired commercial pilot who has undergone a
cholecystectomy 2 days ago. He has à history of COPD, DM type 2, and HTN. He is
on insulin sliding scale QID due to his poor blood glucose control r/t lifestyle and
diet choices. After his wife suddenly passed away due to breast cancer he started
eating in fast food restaurants and stopped exercising. He has 3 incisions in his
abdomen, and a Jackson Pratt drain in place that is to be removed today as it has
only drained 20 mls of greenish bile in the last 24 hours. He has an IV infusing in
his left hand of D5NS infusing at 75 mil/hr, and is still c/o nausea. He has not taken
any of his clear fluid diet today and states “Gravol isn’t working for me’. He has
had no BM for 3 days and his abdomen is slightly distended.

Medications:
Salbutamol Inhaler 2 puffs Q6h
Atrovent Inhaler 2 puffs Q6h
Regular Insulin sliding scale QID
Gravol 25-50 mg IV/IM Q6H prn
Hydromorphone 1-2 mg SC Q3H prn pain
