April is an RN who has just been transferred to the medical-surgical unit after working 5 years in the OR. She’s been learning to organize and manage the care of four to five patients during her 12-hour shift. The majority of her patients are complex and high acuity. April has just been informed that a new patient, 65-year-old Mr. Barnes, will arrive from the recovery unit in 5 minutes, following a total hip arthroplasty. At the same time, April receives the order to administer a blood transfusion to 50-year-old Mrs. Gouter, who is postoperative day 4 following an open reduction internal fixation of the right femur.
The nurse delegates to the nursing assistant, to take the first set of vital signs on Mr. Barnes and record them in the patient’s chart, and she instructs her to report any unusual findings immediately to any staff nurse. Then she goes to take care of Mrs. Gouter.
