In the United States, there has been a steady evolvement in the process of documentation within the healthcare industry. We have evolved from memory, to paper, to a hybrid of paper/electronic and have ultimately moved to fully electronic for almost all documentation for patient visits. This evolution has allowed for greater continuity of care for patients when seeking care within multiple departments (i.e., clinic, lab, radiology, oncology, dermatology, etc.). Does this same continuity of care apply to patients seeking care at multiple facilities that are not within the same organization? This question is often up for debate and why Community Health Records have been explored. Community Health Records (CHR) are a software tool that allows qualified health care providers to access patient records from multiple hospitals and medical labs throughout a community. CHR’s also allow for documentation to take place at the point of visit and can be seen in real-time at a different organization.
