A client with a history of anxiety presented to the ED with the complaint of feeling suicidal.  suicide precaution with one-on-one observation started and the security officer was at the patient’s bedside. After several hr, the security officer left the bedside briefly. when they returned to the patient’s room, found him unresponsive, cyanotic, and hanging from the cardiac monitor with the monitor wires wrapped around her neck and not able to be resuscitated.
