Smokes two packs of cigarettes per day
Diagnosed with Chronic Obstructive Pulmonary Disease in 2011
Home Medication
Flovent by MDI 200 mcg 2 puffs daily
Ventolin by MDI 200 mcg 2-3 puffs daily PRN
Lasix 40 mg PO daily
