FH: Grandmothers had “crippling” arthritis.Â
PE: remarkable for bilateral ulnar deviation of her hands and soft, boggy proximal interphalangeal joints. The metatarsals of both of her feet also exhibited swelling and warmth.Â
Diagnosis: rheumatoid arthritis.
