Scenario Group #1:

What were the main problems that you identified?

Ertha initially presented with cognitive dysfunction. Initially she showed signs of mild agitation and depression due to the move and decline in Henry’s health.  Ertha was also showing clinical signs of Alzheimer’s with memory loss and difficulty with daily activities such as bathing and taking medications.  After Henry’s passing Ertha’s symptoms worsened and her condition continued to deteriorate to a point where intervention and placement in a nursing facility was necessary.  

Explain the nursing management considerations for this patient.

Ertha must be handled with care and empathy.  Ertha is incapable of cognitively understanding her situation and condition.  The nurse must respect and anticipate Ertha’s behavior to show fear, confusion, and inability to cope and care for herself.  The RN must plan safety measures for staff and other residents due to Ertha possibly hitting, and safety for Ertha as she could unintentionally harm herself.  In addition, ensuring medication administration, nutrition, hygiene and comfort measures are provided. The RN will want to include family with Ertha’s care especially while Ertha maintains the ability to remember family and may respond better to their direction.

Alzheimer’s can become a very debilitating condition.  Not only does it affect memory but also causes mood and behavior disturbances.  Patients with dementia may exhibit unpredictable behavior that at times may be violent and self harming.  The patients with mild symptoms are often frustrated with their condition and have depression. As the disease progresses they are often unable to care for themselves and require assistance with daily tasks including bathing, feeding, medications and overall daily living.
