Diagnosis (Nursing): 

What two nursing diagnoses is the patient at risk for or are actually happening? 

 

Planning:

Sally asks “which of these five wishes is most important to focus on right now?”.  “I am so overwhelmed”.  Give your choice and rationale for choice.

 

Intervention:

It is 7am at the time of your visit. You notice that Sally has taken 5 doses of her PRN Roxanol for the past two days in a row and she is currently rating her pain an 8 and asking for an additional dose of pain medication right NOW. In less than 5 sentences, describe an  INTERVENTION for Sally.

 

Evaluation:

Sally reports that the hospitalist during her last admission said that “there is nothing more we can do for you”. You have spent time educating her about the benefits of hospice care.  Write one statement that Sally could make that would indicate that she has understood your teaching.

 

Home nurses often carry a bag with supplies and tools needed to provide nursing care in the home.  They use “bag technique” to prevent and reduce the transmission of infections. What is one way that you can imagine (or look it up) that the home nurse keeps her bag “clean” in people’s homes?
