A nurse has administered a medication IV instead of IM route by accident. Appropriate reporting has been complete. 

 

The patient is recovering from food poising, suspicious it was caused by a local restaurant. The patient starts naming and shaming the local restaurant on ‘Facebook’. The patient tags the nurse in the discussion, as the nurse thinks the discussion is private who inadvertently provides information that more community members are in hospital with similar conditions. The family members of the patients in hospital make a formal complaint to the hospital. 

 

As a result of the medication error the nurse in charge has arranged a peer in service to educate the ward on IM injections.
