a. There is an increase in central line infections on your unit, and the new policy after review of the literature is to cleanse with chlorhexidine for dressing changes.

 

b. Wound rounds are done every week. The wound nurse reviewed the literature and decided to photograph every wound to document wound progress. She is talking to the alert and oriented patients about the photographs. She is not talking to the families of patients with dementia.

 

c. The medical director wants to look at the effectiveness of the regular strength influenza vaccine compared to the high dose influenza vaccine in his nursing home patients. He speaks to the patients and families and obtains consent for the vaccine. 

 

Which one of these scenarios would be considered QI, and which ones would be considered clinical research? Are there any ethical concerns in these scenarios? What are they? How would you go about eliminating these concerns?
