Rani Kapoor is a 48 year old
Rani Kapoor is a 48 year old Punjabi lady who was born in India and moved to Southern River, Western Australia, with her husband Rishi, who is 50 years old. The pair had an arranged marriage in India right before they migrated to Australia in 1998. Rani and Rishi’s families were initially very unhappy about the pair’s migration to Australia but have since managed to visit once in 2003. The couple has two children, an 18 year old daughter, Sheela, and 21 year old son, Jermiah. Sheela and Jermiah attend university and work part-time to help with the family finances. Rani does not currently work, and Rishi is the sole family income provider. He works as a truck driver. Rishi is away for three days while driving the truck, and then he returns for two days before leaving again. Rishi sleeps in the truck while he is away and rarely has time to engage in physical activity. The Kapoor family are active members of the Sikh community. While Rani joins in cooking traditional foods at the temple on Sunday, Rishi prefers to buy hamburger meals and desserts from the fast-food shops. Rishi also likes to spend his non-working days resting on the couch, watching TV, and meeting his friends at the local cafĂ© or temple. Rani visited their family General Practitioner today with Rishi to discuss her concerns about his Type 2 Diabetes Mellitus. Rishi states that he is not concerned about his diabetes, but Rani is very concerned and says that Rishi eats food that he likes that are high in sugar and carbohydrates, not the foods that the Diabetes Educator has recommended that he eat. Rani stated that Rishi very rarely checks his blood glucose levels, and when he does, the blood sugar level results are very high (>15 mmmol/L). Rishi apologised to the doctor as he felt they were wasting his time but said he had agreed to the visit to “keep Rani happy”. Rishi’s father also had type two diabetes and had had a right lower leg amputation because of severe ulceration. The doctor has asked his Practice Nurse to speak to Rishi to assess his understanding of his health. Identify four (4) functional health assessment areas that a nurse should focus on for Mr Kapoor and say why these are being selected. Support your statements with evidence-based information. Nurses can recognise areas of a patient’s health that may need further exploration by reviewing information from a health history. Identify two (2) other questions that the nurse could ask Mr Kapoor to gather additional detail about his diabetic status. State the nurse’s rationale for asking these questions.
