Mr Jim Franklin is a 71-year-old male, who is preparing to return home from hospital following a Cerebrovascular Accident (CVA). He has spent the past two weeks in the John Flynn rehabilitation unit, on the Gold Coast. Although Jim did not experience any long-term dysphagia or dysphasia, he does have some mild to moderate weakness in his left leg. His previous medical history includes, atrial fibrillation, osteoarthritis and he has had previous bilateral knee replacements. He currently takes regular Panadol osteo, aspirin, frusemide and amlodipine. You are part of the discharge team and when assessing Jim and reviewing his case, you find that for Jim to safely mobilise independently he requires the support of a wheelie walker. You notice that when speaking to Jim about his mobility he seems to get upset about the topic and you also note on his file that the nursing staff have stated that he doesn’t like using his wheelie walker much and prefers to use the furniture and rails to balance as he moves around the room. Jim lives alone, after losing his wife 12 months ago and before his CVA was very active within the retirement village his unit is situation in. He has only one son, who is currently living in Brisbane, but is planning to relocate to Perth next month. As part of the discharge planning team and in helping oversee Jim’s discharge home, discuss two areas of care that Jim will need assistance with at home and identify what members of the interdisciplinary team that may need to be involved.
