Discuss the goals of engaging in advance care planning with patients and their families. In this description there must be evidence of the subcomponents within the wellness model (specifically, functional independence, self-care management of chronic illness and disability, positive outlook, personal growth, and social contribution). There is evidence from prospective studies and randomized trials, that Advanced Care Planning (ACP) has significantly improved multiple outcomes, particularly for patients with serious illness. Describe at least one of these outcomes in detail, utilizing supporting data to justify your choice (reference at least one peer reviewed research study and cite per APA 7th addition). Choose one of the frameworks for serious illness discussion or advance care planning discussed in class (POLST, 5 Wishes, ME ACP form) and explore the basic tenets of the approach and main considerations for completing with patients. (Address a – c below) What do patients need to know to make decisions in advanced care planning? What are the clinician’s responsibilities during the decision-making process? Describe at least 1 cultural and 1 spiritual issue that the clinician may need to consider in this discussion. Discuss the ethical considerations that are important when assisting your patients with making complex and advance care planning decisions (Touhy & Jett (p. 417 – 419; and other resources posted). Reflect on the experience of engaging in this discussion with either a real patient, a family member or friend. What comes up for you? Where are your gaps in knowledge as a nursing student? What do you hope to learn and/or to implement in your practice?
