respond to discussion 5. ACE- inhibitors are medications that help lower blood pressure by preventing the production of vasoconstrictor angiotensin II, and by blocking the breaking down of bradykinin, a peptide that increases production of vasodilators; however, increased amounts of bradykinin can cause dry cough and angioedema, which can be life threatening. Patients need to know to report symptoms of swelling of the face, lips, tongue, larynx and abdominal pain, and when to seek immediate medical attention. A decreased angiotensin II level decreases the secretion of aldosterone, leading to lower sodium reabsorption, in turn affecting the kidney excretion of potassium. Patient is to be informed of signs and symptoms of hyperkalemia. Patients need to be educated on the importance of limiting foods high in potassium. Provide the patient with a list of low-potassium foods to select and high-potassium foods to avoid. ACE-inhibitors are teratogenic, can cause birth defects and thus should be avoided during pregnancy. Patients in child-bearing age to be informed of the importance of informing the provider when pregnant or plan to become pregnant. Patients to be educated on the importance of monitoring blood pressures, medication compliance; and to be counseled about the benefits of regular exercise, healthy diet, quit smoking and limit alcohol consumption.
