After the passing of her husband, she now lives alone in the two-bedroomed bungalow that they used to share. Since her husband’s passing, Carol has spoken about how she fears she cannot get by without him and that she can’t manage by herself. She has also said she is now incredibly lonely as he was her main company, and they only used to go to the occasional coffee morning or community event, which he drove them to. Carol’s family have expressed their concerns to you as one of her support workers, that she is becoming withdrawn and having a confidence crisis in her ability to live and manage alone. : After speaking with Carol at length, she has decided that she would be keen to get access to a mobility scooter so that she can travel further from her home in order to socialise. With the NDIS in place, how could she gain funding for a mobility scooter? Research into if a scooter would be funded under the NDIS. Below is a recent independence assessment regarding Carol: Aspect of independence Observations/Notes Goals/Requirements Medication Organises her own medication into a daily pill organiser but relies on her daughter to pick up her prescriptions and drop them off Arrange for the local pharmacy to drop medication to Carol if she wants this Diet Carol doesn’t tend to do much fresh cooking. She eats lots of sandwiches and ready meals but also loves eating fruit as a snack. She doesn’t drink any alcohol but loves a cup of tea at any opportunity Carol’s diet is not too bad though there is not a lot of variation. Support could be given as to the preparation of simple, fresh meals she can do independently. Provide information to Carol about food delivery services as well as centres she can go to for lunch/dinner Socialising The family drop in from time to time, but other than this, Carol largely stays in the house as she cannot drive. She does venture out to the local food store to pick up groceries, only when it is essential Inform Carol of mobility services available to her, such as scooter access and pick up services as well as local opportunities for socializing such as clubs, events, etc. Mobility Carol cannot stand for long periods of time but does move around her house slowly with the aid of a walking stick. She can get herself in an out of the shower/bath but says it does cause her pain to do so she often just has a brief wash at the sink. She goes to the shop on her own but doesn’t undertake targeted exercise Ensure her home is as suitable as possible in relation to her mobility. Encourage Carol to continue to undertake gentle exercise Activities and hobbies Carol loves doing crossword puzzles and has a great knowledge of the world, having been a teacher until she retired. She also enjoys knitting and donates baby clothing she has made to a local charity Encourage Carol to continue doing the hobbies she enjoys. Provide information to Carol about local hobby/interests groups that she could join for different activities and for an opportunity to socialise. Hygiene and physical selfcare Carol prides herself on her appearance so ensures she gets washed, brushes her teeth, gets dressed smartly and does her hair and makeup on a daily basis. She has a mobile hairdresser visit once a month to cut and set her hair. She cannot bend over to tend to her feet, however, and as a result, her toenails and soles need attention Speak with Carol regarding visiting her doctor/ a podiatrist in order to get her feet cared for appropriately.
