Chapter 12: Bipolar Disorders Have you been around someone who is in a hypomanic state? A person in a hypomanic state can be someone who is highly entertaining, talkative, productive, pleasure-seeking, and fun to be around. But then the condition can cross a line, causing the individual to become more agitated, need little sleep,or enter into risk-taking and aggressive behaviors. Do you think someone who is in a hypomanic state should be pushed to seek psychiatric treatment if he or she doesn’t feel the need for it? What would be an ethical implication of pushing someone who is feeling good and invincible into treatment: Chapter 13: Suicide Suicide is an emotional and painful subject, but it is often hidden from public view. Families often don’t want to acknowledge or talk about it. It is difficult to know how to respond to suicide. It often gets talked about more when a public figure either attempts or commits suicide. Think about a public figure who has attempted or committed suicide. What was your first reaction? How did it make you feel to hear this news? Describe the impact that this news had on the public. Have you known anyone in your life that you suspected of having a personality disorder? If so, think about what he or she did that made you suspect this diagnosis. In your notebook, describe the traits you have observed and what personality disorder you think might be present. Identify the challenges this person presented in your life. B. Staff splitting is a common reaction to the chaos created by someone with borderline personality disorder. This occurs when a patient picks up on conflict or vulnerabilities in the staff and uses these to create more conflict. For example, a common scenario occurs when a patient complains to the day nurse about the “nasty” night nurse, then tells the day nurse that he or she is the only one who understands. The outcome of this can be that the night nurse wants stricter limits on the patient, while the day nurse becomes more sympathetic toward the patient. The outcome can be disagreements between the shift nurses and the creation of more tension. Perhaps the patient has picked up that the day nurse has low self-esteem and is eager to have someone compliment him or her. Have you observed staff splitting? Have you been a participant in it? If so, describe the situation from both sides. Discuss how this developed. Identify warning signs and how to counteract it when you see it. How has the staff contributed to this developing problem?
