Cultural Competence – A Story about an Asian-aged female patient Mrs. Z, age 78 with a diagnosis of osteoporosis when she was 50 years old. She was admitted six weeks ago to the Medicine unit for a fracture to her left hip. On admission, there were no family members present. Previous notes indicate that she is a widow with no children. As a result, the staff had difficulty ascertaining a comprehensive assessment to provide optimal care as Mrs. Z speaks and understands very little English. At shift change, the transfer of accountability to May, the incoming nurse, indicates that Mrs. Z is non-compliant with care and treatments. As a result, nurses on the unit are reluctant to be a part of her care team. May is a new nurse on the unit, and she has some experience with the Asian culture and decided to offer Mrs. Z a cup of hot water prior to initiating care. Her rationale was that in the Asian culture, it is believed that having warm water in the morning aids in promoting blood circulation and the release of toxins. On observing the warm water May had, Mrs. Z immediately sat up from her bed, and although May could not verbally communicate with her, her gestures suggested that she was grateful. After having her warm water, with the use of gesticulations, Mrs. Z expressed that her care has been subpar since she was admitted. May was able to utilize non-verbal cues_____ to provide care for Mrs. Z. For the remainder of the shift, Mrs. Z took her scheduled medication, and was also compliant with care, after which she participated in physiotherapy. As mentioned earlier, Mrs. Z had been admitted six weeks prior, and the team had great difficulty regarding the approach to care. The following day, May was approached by her manager, who commended her. The healthcare team held a meeting where May shared her strategies that were effective in building trust with Mrs. Z. A personnel for the Chinese community ____ was also present at the meeting and was able to educate the team on how Asian view healthcare. A.
