Case Study scenario: Mr. J. is an 82-year-old African American man who was diagnosed with heart failure and end-stage renal disease secondary to hypertension and diabetes. He is on several cardiovascular medications to manage his condition. He is currently receiving dialysis treatments 3 days a week and is on the organ-donation list awaiting a kidney transplant. Mr. J. Has been experiencing chest pain intermittently for the past few days. He has a history of angina (chest pain) and is prescribed medication for management. Three days ago Mr. J. Was found at home unresponsive by his daughter, who called 911 and immediately began cardiopulmonary resuscitation (CPR). Mr. J. was taken to the nearest hospital and admitted to the ICU. He suffered a massive heart attack resulting in a lack of oxygen delivery to his brain. Due to the unwitnessed heart attack, Mr. J. was not a candidate for therapeutic hypothermia management. He is currently intubated on ventilatory support and continuous renal replacement therapy. During neurological assessments, there is minimal response from Mr. J. The interdisciplinary team caring for Mr. J. has arranged a conference with his family to discuss his current status and prognosis. Mr. J. has a large, close-knit family and, despite his illness, has been an active member of his church for the past 30 years.
