SBAR Report from Cath Lab RN for Jimmy Blanc-Valvuloplasty S= Jimmy is a 4-year old boy admitted through ambulatory surgery this morning for balloon valvuloplasty of his aortic valve. We’ve completed a successful valvuloplasty without complication. B= Jimmy was diagnosed with aortic valvular stenosis at age 2; symptoms were noted at 18 months when mom noticed he became short of breath and easily fatigued when first learning to walk. His pediatrician found a murmur in the right sternal border, 2nd intercostal space and referred the family to a peds cardiologist. He has no other medical conditions, was born full-term, and is at the 95th percentile on the growth and development charts. He is developmentally appropriate for age. Allergy to PCN-rash. Jimmy has an older brother and sister who are being cared for at home. A parent is with Jimmy and speaks limited English. A= No murmur was auscultated post-valvuloplasty. I gave him 3mg of Versed and 50 mcg of Fentanyl IV for conscious sedation during the procedure. I had him on a 60% face mask during the procedure, but now he’s on Room Air, satting 98%. Sinus rhythm 95 94/46 RR 24. Hemostasis was achieved after 15 minutes of direct pressure on groin site. Distal pulses are palpable. He has been a little agitated upon waking up but calms with presence of parent who will come up with him. R= I have his orders with our typical post procedure protocol and will bring them up with me. He will be discharged home after 24 hours post-procedure observation. What targeted assessments will be needed? What potential complications could occur?
