Quality of life is an important issue for elderly clients especially when facing the end-of-life. As per the World Health Organization, “Palliative care improves the quality of life of patients and their families’ facing problems associated with life-threatening illness by preventing and relieving suffering through the early identification, correct assessment, and treatment of pain”, (WHO, 2020). Palliative care is end-of-life care services which can be provided in the client’s home. Although most elderly people wish to die in their own home many do not. Some reasons for this happening are older adults not having advanced directives in place that ensures wishes during the process of dying will be carried out even when incapable of voicing wishes. Another factor that determines if a patient will die at home, is functional decline. Older adults who have a functional decline and are impaired for a t least 12 months before death are likely to die in a nursing home (Morrison, 2014). Many older adults do not want to be a burden to family members who will be ultimately responsible for care. As a nurse to support a client regarding end-of-life care in accordance with their wishes I would recommend putting an advanced directive in place. An advanced directive would provide a plan of care for healthcare providers to follow to legally meet the wishes of the client. It makes advocating for the client easier knowing what life saving measures the client wants performed and not performed.
