Ms. S is a 58-year-old woman employed in housekeeping in a local hotel for 10 years who presents now with “much pain in right knee for 1 day.” Subjective L.M. reports being overweight since having children in her 20s and having bilateral knee pain for the past 2 years. No morning stiffness, but pain increases during working day. For last 6 months takes 6 extra-strength acetaminophen (500 mg each) in 3 divided doses daily, but pain persists. One day PTA, walking downstairs at work carrying load of sheets, reports right knee gave way and almost fell. Continued working, but today pain is increased. Able to bear weight and walk to car but unable to work. No history of stomach pain or stomach bleeding. No smoking, no alcohol. Objective Vital signs: Pulse 76 bpm. Resp 12/min. BP 146/86 mm Hg. No fever. Height 64 in, weight 180 lbs. BMI 31. Right knee has no swelling or increased pain on palpation. ROM both hips full on external and internal rotation, abduction and adduction. Unable to flex hip with knee straight (says has not done that in years); hip flexion with knee flexed is 90 degrees and painful on right.
