An 85-year-old male was brought to the emergency department via EMS from an assisted living facility with a history of dementia, depression, and high cholesterol. The staff at the facility reported that the patient had been having increased aggression and sexual behavior towards the staff and other residents. The staff also reported that the patient had been living there for about three months and prior to that the patient was living alone at home. The facility was requesting a psychiatric evaluation and admission before he was allowed to go back to the facility. When the patient arrived at the emergency department, he was ambulatory, alert, and oriented to self and place. The only medication this patient was taking was atorvastatin. I picked this patient to discuss because of the medications that were started by the psychiatrist in the ED while the patient was being boarded waiting placement for acute psychiatric care and how they affected him so drastically. Twenty-four hours after arrival to ED the inpatient psychiatrist rounded on this patient and started him on Ativan, Remeron, and Paxil.
