A patient is recently admitted up to

A patient is recently admitted up to the PACU after coming in through the ER with an initial complaint of angina. The patient’s General Practitioner requested a 24-hour floor admission observation to do some follow-up cardiovascular testing. Upon arriving on the floor the nurse assigned on shift notices during the physical assessment that the IV site placed in the ER seems to need cleaned up and evaluated. Upon further inspection, the patient appears to have a poor capillary refill, pallor, and some swelling in the fingers just distal from the IV site. The nurse plans to remove the IV site placed in the ER setting and replace it with a new site. What pathologic mechanism could possibly be occurring in this patient? Describe it in detail. What might the nurse need to prepare for or be on alert for assessing when changing these IV sites? What could be three pathologic risk factors that would need to be evaluated for this patient?
