A 56 year old male is ventilated
A 56 year old male is ventilated and sedated post respiratory failure secondary to pneumonia. CT of the chest has identified a 6 mm tumor in the patients right mainstem bronchi. A flexible bedside bronchoscope is planned for later this afternoon to assess and possibly biopsy the lesion. As you research your patient you learn that the patient has a diagnosis of IDDM, Diabetes, alcoholism and has had a recent hospitalization (<3months) for a bleeding ulcer. The patients assessment finds: HR 112, RR 22, BP 103/65, Temp 37.2C, Breath sounds are diminished. CXR shows a pulmonary infiltrate on the right with consolidation in the middle lobe. You have been suctioning thick yellow-green purulent secretions. The patient is currently on a vasopressor. Labs are as follows: Na-138mEq/L, K-5.5mEq/L, CL 105 mEq/L, Glucose 154mg/dL, INR 0.4 , Hb 8.5g/dL, WBC 12.7 cell/mL. Assess this patient and report abnormal findings.
