Pressure area measures 2cmx 2.5cm x 5cm. Black eschar is present between 7 and 5 o’clock. Yellow gray slough is present from 8 to 4 o’clock. Undermining present at 3 o’clock measuring 0.25cm. Tunneling noted at 1 and 2 o’clock measuring 3cm. Wound edges are intact from 7-5 o’clock. Wound edges from 8-4 o’clock are mushy. Moderate amount of foul smelling yellowish green exudates present.
