: As you review Mr. Jones’ social and medical history, you note several strengths and weaknesses that play a role in how Mr. Jones is able to connect with you as a provider, how he understands the surgical risks, and his ability to cope both mentally and physically post-surgery. What are those strengths and weaknesses? As a young adult Mr jones began to have impaired vision during night time, which progressed with narrowing tunnel-like vision into adulthood. During this time , he was diagnosed with choroideremia , a rare genetic condition causing progressive vision loss, he learned he would likely lose his vision completely as he ages. After receiving this news, he became depressed and for a year, mostly stayed at home, refusing to see the clinicians to follow up his hypertension and diabetes. his wife remained supportive and tried to get him to socialize with no luck. Her own clinician found a support group for her husband and even though Mr Jones refused to go for support group meetings , Mrs Jones wnet on her own, she befriended several individuals with Choroideremia with varying levels of vision. These individual wanted to help and decided to make a home visit , After several visits, he realized he could still lead a meaning full and active lifestyle even with visual impairment. Ove the course of several years Mr Jones attitude changed , he accepted his progressive vision loss. He became a committed member of the support group and tool multiple classes on how to live as a visually impaired individual.He learned Braille in anticipation of his progressive visual loss and restarted visits to his clinicians and got his diabetes and hypertension under control. At one of these visits he was also diagnosed with prostate cancer . Despite this, he felt self sufficient and in charge of his own health. By the age of 60 he become almost completely blind. Mr jones is now 70 yrs old and reffered to surgeon for a prostatectomy . Dring his clinic appointment with the surgeon , it was clear the surgeon was frustrated with Mr Jones . the surgeon usually uses visual aids to explain to his his patients the procedure to be performed but coukd not rely on them with Mr Jones , He tried to explain to Mr Jones the procedure and potential complications if his cancer spread.When Mr Jones asked further questions on the procedure , the surgeon become increasingly frustrated with his inability to communicate . one questions MR jones wanted a clear answer on was whether the procedure caused sexual dysfunction.
