Present Status: Identifying material such as name, age, client’s self-presentation and within-interview behavior. B. Family & Home Background: Structure, origin, and interrelations of members as described by client, social, economic, and cultural status of the family. C. Health & Physical History: Include any serious illness, injury, operations, physical defects, physiological maturity, and apparent ability to meet the physical demands of the environment. D. Social History: As indicated by the nature and extent of activities; group and individual hobbies and recreation; number and kind of friends and acquaintances; relationships with friends and significant others (where appropriate); sexual history. May include a description of the client’s typical day. E. Education History: Review pertinent educational, vocational, and training experiences. F. Work History: Part-time and full-time occupational experiences, level of interest, and satisfaction. G. Plans & Goals: Vocational and educational; life goals; nature and apparent commitment to goals, relevant interests, special abilities, and aptitudes. GOALS Provide 2-3 GOALS for counseling and discuss how they will support the AIM State each GOAL in operational terms and how each will be measured (see Chapter 5 of your Treatment Planning text) STRATEGIES For each GOAL above, provide at least one STRATEGY (therapeutic approach) For each STRATEGY, discuss how this STRATEGY will help the client achieve the GOAL (be specific here – see Chapter 6 of your Treatment Planning text) TACTICS For each STRATEGY, identify at least one TACTIC (technique) that will be used to implement the STRATEGY.
