`At the end of an 8-week Clinical

`At the end of an 8-week Clinical rotation, how does the use of a Nurse Report/Hand-Off Sheet versus a Concept Map increase clinical judgment skills as measured using the Tanner Model Evaluation Tool in twenty-two Associate Nursing Students? The university always uses care maps students. Care maps consist of patients initials three medications IV sites past medical history treatment and then assessment the form cannot be used throughout the shift and then just turned in. I used care maps my first week with my students and then I made a nursing report/handoff sheet. Students then prior to clinical shifts were made to look up patients and use their report sheets throughout the shifts to write down vital signs, changing of labs, I&0’s, assessments, medications, interventions, documentations, and notes throughout the shifts and then give a sbar handoff considered as bedside reporting verbally in front after clinical was over in front of class. I as the instructor used the tool the Tanners Model to judge each student in their Clinical judgment skill each week to see how they were evolving with the choice of tool given. During the 8 weeks of clinicals students were given 3 care maps and 5 nurse report sheets. Every time they gave a SBAR sheet at the end of clinical. Each student filled out at the end of clinical a Care Map Tanner Rubric and Nurse Report/handoff sheet Tanner Rubric to see what tool helped them Think like a nurse and achieve better Clinical reasoning, Clinical Thinking and Clinical judgment skills. The Results were very much Subjective that the student preferred the Nursing report hand off sheet as this prepares them for floor nursing and helps them Think clinically. They also like the fact they could practice using the tool and bedside reporting before they became a floor nurse to become more proficient in there terminology as a floor nurse. There was Not one individual out of 22 students who did not like the Report hand off sheets over Care Map sheets. I as the instructor also saw a huge improvement in Clinical Reasoning and judgment skills through each session after clinical using the Tanner Rubric and listening to the bedside reporting. For my project which is already done my mentor doesn’t like my objectives as they are student led and not instructor led. Specific knowledge and skills you want to acquire during this experience. Written as measurable objectives so that you, the mentor, and your faculty can evaluate if you have met the objectives and assess your practicum outcomes) I need to have them in 4 SMART learning objectives.
