Page of 2 ZOOM NURS3012 Care for People with Chronic & Complex Health Needs (2024) 1 Taylor : 11yo – (pronouns her/she ) • Parent: mark 42yo – surveyor , Mother , Janet died 2 years ago from breast cancer • Lives in Seaford Rise, Adelaide, SA General medical background • Diagnosed at 3years old with juvenile rheumatoid arthritis. • Her hands and her feet and cause her pain and impaired mobility especially during a flare up. • During the cold weather Taylor experiences increased joint pain • No allergies Current situation Medications • Paracetamol 450mg 4-6 hourly • Humira (adalimumab) 40 mg fortnightly in Pre-filled Syringe) currently administered by Mark. • Naproxen 125mg twice daily Taylor has a group of friends at school who seem to understand about her mobility limitations, but she often needs to sit and watch because of joint pain during recess and lunch. Taylor enjoys playing netball and would like to join the local netball club the Seaford Storm but is not sure they would let her play because of decreased mobility. At school next term all the year 8 students are going on a two-night camp in the Flinders Ranges. The focus of the camp is outdoor activities including orienteering and sleeping in tents. Taylor is excited about the camp but is unsure how she will manage all the activities, sleeping on the ground and her medications. Taylor sees her rheumatologist every 6 months who is monitoring her growth, development, and level of joint inflammation through blood tests and other monitor tests. • . Taylor also states she would like to find out if there is anything that she can use to make sleeping more comfortable with her painful joints. Taylor would like toTASK Record an oral presentation that provides one of the 3 (three) available cases and their family with the following information, Using the identified self-management needs, address 2 (two) opportunities for building on the case person’s health literacy. Identify a range of current information/community support services relevant to the needs and personal circumstances of the case person. The services must be appropriate for the case, local and support self-management. The Identify strategies must focus on avoiding or / reducing hospitalisation episodes. SCENARIO You are the RN working n the community You have been asked by the case person and / or their family to address and present them with, options to support their health literacy for self-management needs. Assume that the person is receiving Commonwealth support for which they are eligible, they will be using this information to decide how they want to use that funding depending on the information you supply. That is why a ‘range’ of options are required. Assume that the services will be reviewed at some time in the future. REQUIRED FORMAT Video recording of between 4 (minimum) and 5 minutes (maximum).
