Case Study for Health Promotion Project number 1 Increase Breast

Case study Gertie Martin is a 24-year-old female patient and admitted into the hospital surgical ward post appendicectomy two days ago. Gertie has paraplegia and is a wheelchair bound user. Gertie has bladder and bowel incontinence and is obese. Gertie usually eats take aways mostly and her diet usually consists of pizza, fried chicken, chips, burgers, sausages via home delivery service and hardly consumes any vegetables nor fruits. Gertie lives at home alone and has supported in-home supports via the National Disability Insurance Scheme (NDIS) scheme. Since admission to the surgical ward Gertie is being nursed on a normal mattress and has been lying mostly in a supine position for the past two days as Gertie depends on others to change the position. Sophie (Gertie’s Support Worker) assisted Gertie to the shower in the morning and noted that Gertie has pressure injuries on her body and reported this to the nursing staff. On examination the nurse caring for Gertie notes the following: Blood Pressure: 130/ 80 mmHg Heart rate: 88 bpm (regular pulse) Respiratory rate: 18 bpm Temperature: 36.1°C Oxygen Saturation: 96% (Room air) Capillary refill: less than 3 seconds Level of Consciousness: Glasgow Coma scale 15/15, PEARL, Alert and orientated to person, place, and time. Skin: stage 1 pressure injury on the back on left elbow (olecranon), stage 3 pressure injuries on the right elbow (olecranon), stage 2 pressure injuries on the right shoulder, stage 4 pressure injury to the left heel, deep tissue pressure injury to the right heel and unstageable sacral pressure injury. Pain score: 6/10 sharp pain on the elbows and right shoulder Blood glucose level: 8 millimoles/ litre Weight: 120 kilograms Height: 175 cm BMI: 39.2 Past medical history: Nil Allergies Allergies – nil known allergies Lifestyle habits Smokes 10 – 20 cigarettes/day Consumes one to two bottles of wine/day. Occasionally smokes Marijuana Familial History: Mother – hypertension, Diabetes mellitus, Chronic Obstructive Pulmonary Disease (COPD) You are required to design a five (5) minute education session , aimed toward peer nursing students. The education session needs to be completed via Powerpoint presentation with voiceover. In the Powerpoint presentation, address the following items: 1. Introduce yourself to the pseudo-audience (peer nursing students). Provide an introduction to your education session, this should include the objectives/expected learning outcomes of the session. 2. Explain the primary aetiology and pathophysiology of pressure injuries and integrate the information from the case study to support your explanation. 3. Explain the stages of pressure injuries and integrate the information from the case study to support your explanation. 4. Using relevant links to the patient in the case study, explain one (1) evidence-based nursing intervention and one (1) collaborative (interprofessional) intervention that could be applied to prevent and/or manage the pressure injuries 5. Briefly explain how the evidence-based nursing intervention and collaborative (inter-professional) intervention will address the underlying pathophysiology of this health challenge 6.
