The attending physician approaches you to tell you that they just visited the patient who was breathing at a rate of 36 and appeared distressed. The doctor requests you give ativan first. How would you respond? I would respond to this request from the doctor by ensuring that we have evaluated the patient’s pain level already in case these symptoms are being caused by something other than anxiety. If she hadn’t had a pain assessment done yet, I would do so and then potentially suggest morphine or another pain medication to control her discomfort. A family member approaches you distressed by the onset of their loved ones “gurgling.” They ask: “Is there a medication you can give or will you suction the mucus?” How do you respond? I would respond to this family member by first educating them on the gurgling sounds. I would tell them that this is generally called a “death rattle” which is a noisy breathing pattern that can occur in people who are near dying (van Esch, H., et al., 2020). It happens due to mucus and saliva collecting in the throat when the individual is too weak to clear their secretions (van Esch, H., et al., 2020). I would also let them know that it is a normal part of the dying process and their loved one shouldn’t be feeling any discomfort. I would inform them that suctioning is not usually recommended because it can be uncomfortable for the patient and may not be effective, but there are medications available to help reduce the secretions (van Esch, H., et al., 2020). Identify and discuss first line treatment of dyspnea. Include both pharmacologic and nonpharmacologic interventions. The first line of treatment for dyspnea with pharmacologic interventions include opioids like morphine to reduce the sensation of breathlessness and to provide relief. Benzodiazepines like Ativan may also be used if anxiety is contributing to their dyspnea. (Rogers, B., Minteer, J., 2023) This can help calm the patient down and slow the respirations. Non-Pharmacological interventions may include elevating the head of the bed or using pillows to prop up the patient to help ease breathing, using a fan to have the breeze give the patient a reduced sensation of breathlessness, oxygen therapy if the patient is hypoxic, and keeping a calm and quiet environment to reduce anxiety (Rogers, B., Minteer, J., 2023).
