After reviewing the updated milestones assessment, indicate the MIlestone domains and present levels for which progress or acquisition of new milestones was achieved. Sample format: Milestone Domain-Mand (Requesting): The learner met full criteria for all five milestones in Level 1 and acquired two new milestones in the second assessment on Level 2; M6 (milestone 6)-’20 mands for missing items without prompts’ and M7-spontaneous mands for others to emit 5 different actions or missing actions needed to enjoy a desired activity. The area demonstrates an increase from 5 scored points to 7 scored points, which would place the learner on level 2-mand repertoire at approximately 18-30 months. After reviewing the barriers assessment-indicate at least 5 areas with present levels, in which the learner made notable improvements. Sample format: Sample format: Barriers Area-Behavior Problems: In the second assessment the learner’s score improved from a score of 4 to a score of 2, meaning th learner may emit a variety of minor negative behaviors daily (e.g., crying, verbal refusal, falling to floor) but no longer engage in severe negative behaviors that are a danger to themself or others (aggression, self-injury). Lastly, the team will review the transitions assessment and will indicate 5 areas with present levels, in which the learner made notable improvements. Sample format: Transition area-Toileting Skills: In the second assessment, the learner’s toileting score improved from a score of 1-wearing a diaper all day to a score of 3-indicating the learner is now daytime bladder trained, uses pull ups, has occasional accidents, needs prompts and assistance to complete the toileting routine.
