Patient Introduction Location: Pediatric Unit Time: 07:00 Report from shift nurse: Situation: Brittany Long is a 5-year-old female with a history of sickle cell disease diagnosed at 6 months old and treated with regular folic acid supplement. She was brought to the emergency department (ED) last night by her caregiver. Background: The caregiver stated patient has been complaining of right lower leg pain over the last 2 days. She has not had an appetite in the last 24 hours but is taking small amounts of oral fluids. She has had pain crises before, mostly managed at home with acetaminophen and ibuprofen. She has been hospitalized twice, once at age 4 years for a vaso-occlusive crisis (VOC) episode and once at age 3 years for a fever. Assessment: She had pain medications of intranasal fentanyl and IV ketorolac in ED at 06:00. She is asleep currently but is easily arousable. Since she came up from the ED, she has been taking small amounts of oral fluids. Child life consult initiated. The team has just rounded. When examined, she rated her pain as a 6 on the FACES scale and did not want them to touch her leg during assessment. They provided new orders for pain medications. Recommendation: Continue to assess pain, to administer meds and IV fluids, and to encourage deep breathing and ambulation.
