Two priority needs are to be identified and a rationale provided for each. Students are also required to provide an overview of nursing interventions for each priority need and conclude with anticipated outcomes for the person in the selected case study. Task Instructions Step 1: Case Scenario Giovanni Rossi is an 85-year-old retired dairy farmer living independently in Coffs Harbour, New South Wales. Originally from Italy, Giovanni moved to Australia in his early twenties. He has limited proficiency in English, which sometimes affects his ability to communicate effectively with healthcare providers and others in the community. His wife, who took care of business, has recently passed away, which has had a significant impact on Giovanni’s emotional well-being. He deeply misses the routine and satisfaction of working on the land, which was his identity. Giovanni has children. However, the relationship deteriorated after Giovanni stopped providing them with financial support. This lack of support angers him and has left him feeling isolated. Giovanni has a long history of hypertension and diabetes. However, he does not consistently take his medication. He has not visited his General Practitioner since his wife’s death. Recently, Giovanni was found by his neighbour vomiting in his garden after attempting an overdose of his regular hypertension medication. The neighbour called an ambulance, and Giovanni was taken to the hospital. His neighbour mentioned that they have observed Giovanni not caring for himself since his wife passed. He presents as neglected and is heard talking out loud to his deceased wife about his unhappiness. They have also discussed his strong desire to avoid going into a care facility for older adults. Giovanni is currently in the medical ward of the hospital and has been referred to the mental health team due to the identified suicide attempt. With the assistance of an interpreter, he was assessed as having the following concerns. – Feeling isolated and very lonely – Thoughts of suicide, however, he states, “I’m so pleased it didn’t work as my wife would be very upset with me, and it is against my religious beliefs.” – Has a reduced appetite and has found it difficult to prepare meals for one – Sleep patterns are irregular, with feelings of tiredness; he naps for around 1 hour in the morning and 2 hours in the late afternoon – Desire to remain in own accommodation and be financially secure – Is feeling a loss of identity since retiring – He is finding it challenging to communicate effectively, as English is his second language – He would like to take better care of himself. The Psychiatrist has assessed his suicide risk to be significantly reduced, with adequate protective factors, and they have prescribed Sertraline 50mg once daily with a review with the Psychiatrist fortnightly and weekly home visits by the nurse. Review the case scenario to identify the holistic biopsychosocial needs of the person in the scenario. Step 2: Introduction a brief introduction, providing a concise purpose and overview of the clinical case scenario selected. Step 3: Priority areas Identify two priorities for the person within the selected clinical case scenario, providing an evidence base rationale for why these are priorities. Step 4: Nursing interventions Identify nursing intervention(s) that could address the two identified priority needs.
